A safe home can be the beginning of positive change

Housing difficulties can have a significant impact on mental health and wellbeing. Worry about losing your home, homelessness, unsafe accommodation or uncertainty about where you will live can affect every part of daily life.

LilyAnne’s Wellbeing does not provide or allocate housing. Our role is focused on mental health, emotional wellbeing and helping people understand where they may be able to access appropriate support.

For supported accommodation, we work alongside Precision Acres, a registered charity providing supported housing through professional referral pathways.

Precision Acres

Precision Acres provides safe, supported accommodation for people facing homelessness, housing insecurity, domestic abuse and other circumstances where additional housing support may be needed.

Accommodation is accessed through professional referral pathways rather than a public walk-in housing service. Each referral is considered individually, taking account of housing need, support requirements, risk information and available accommodation.

Precision Acres currently lists supported housing in areas including Hartlepool, Middlesbrough, Stockton and Durham, subject to suitability and availability.

Precision Acres focuses on housing and supported accommodation.

LilyAnne’s focuses on mental health and emotional wellbeing through counselling, listening support and peer support.

Together, housing stability and emotional support can help create stronger foundations for recovery, independence and wellbeing.

SUPPORTED ACCOMMODATION

Who may be able to access support?

Precision Acres states that its accommodation may be suitable for people in a range of housing circumstances. All placements remain subject to assessment, suitability and availability.

Homelessness

People experiencing homelessness or at risk of losing their home.

Unsafe accommodation

People leaving domestic abuse or another unsafe living environment.

Mental health needs

People whose housing needs may sit alongside mental health or wider support needs.

Leaving other services

People leaving hospital, supported accommodation or other structured services.

Care experienced adults

Young adults transitioning from care who may require supported accommodation.

Leaving custody

People leaving prison or working with probation services who require housing support.

ACCESSING ACCOMMODATION

How the referral process works

Precision Acres does not operate a public walk-in accommodation service. Housing is accessed through an appropriate professional referral.

Speak to a professional

Contact your council housing team, social worker or another appropriate support service.

Referral submitted

The referring organisation provides the relevant assessment and support information to Precision Acres.

Referral reviewed

Precision Acres reviews the person’s housing needs, support needs, risk information and accommodation availability.

Placement progressed

Where suitable accommodation is available, Precision Acres works with the referring organisation to progress the placement.

MENTAL HEALTH SUPPORT

Housing is one part of feeling supported

Precision Acres identifies LilyAnne’s Wellbeing as a partner for mental health support. Residents can access LilyAnne’s counselling, listening and peer support services alongside their housing support.

You do not need to be a Precision Acres resident to access LilyAnne’s wider wellbeing services.

FACING HOMELESSNESS?

Contact your local council as soon as possible

If you are homeless, at risk of losing your home or staying somewhere unsafe, contact your local authority housing or homelessness team. You do not need to wait until you are sleeping rough before asking for help.
